"Custom title"?

I've been wondering how does someone put/get them (these "titles" that someone can find over usernames, but not everyone has them). For example, when you go to an user's profile, you can find this:

Custom title: something.

Is it only available for the staff?
I'm confused as hell; thanks in advance.

Custom Forum Titles are usually earned by winning Official LM Events, for example, the winner of the Member of the Month contest gets said title or sometimes in other special contests (getting titles like "Halloween Master", etc).
And yea, staff have Custom Titles too sometimes.

And who knows, goof titles can also lurk around ; )
